Social Prescribing Triage Officer


Initially a 1 year fixed-term contract.  Expected to continue subject to funding

Salary £21,000

Hours full-time 37.5 per week.  Job share will be positively considered


Qube is an independent charity serving the community across North Shropshire.


A new opportunity has arisen at Qube for a Social Prescribing Triage Officer. This brand new role is an exciting opportunity to join our Qube team and help shape the continued development of social prescribing across north Shropshire.


Social prescribing is a non-medical ‘prescription’ which supports people to make positive changes to their lives.


The main purpose of this post is to make an initial assessment of all patients referred to social prescribing assessing the needs of the person and referring them on to the appropriate community or health service.


Based in the Ellesmere GP Practice this role will work very closely with health professionals based at the Churchmere group of practices as well as the Social Prescribing Advisors and Community Development Officers working with the community organisations providing the interventions.


The successful applicant will be educated to degree level or equivalent experience and will have experience of working within the health/care or voluntary sectors.  This post requires an organised, can-do attitude together with the ability to communicate with a wide range of individuals.


This role is full-time and initially a fixed term contract for 1 year with the expectation that it will continue subject to funding.  Job sharing will be positively considered for the right applicants.
